認知症者への基本対応
- 受容（否定しない、認知症者の世界観を受け入れる）
- 傾聴（自分のききたいことではなく、相手が話したいことを聞き出す）
- 怒らない（自己覚知によるアンガーマネジメントで対策）
ナースコールで人を呼ぶ、物音をたてる事例：
用事がないのにナースコールを何度も使用され、時にはゲームのコントローラーのように連打されています。
またベット柵を揺らし音を立てることもありフロアまで鳴り響きます。
介護者は多忙なうえ、頻繁に呼び出されると疲れてしまうので、しだいに放っておいたり、後回しにするようになりました。
ナースコールで人を呼ぶ原因：
・不安な気持ち
自分の存在を忘れられたのではないか
キレイ好きな方で、寝たらおむつを交換してくれないのでは
・頻尿
残尿感
服薬による利尿作用
・認知症
ナースコールの意味が分かっていない
頻回にナースコールを使用しない対策：
１人ベットの上で寂しく寝ていると、周囲の状況はわかりません。本人にとってはナースコールが自分と世界をつなぐ命綱のようなものになっています。
寝たきりのお年寄りは「自分が見捨てられることはない」「いざとなったら助けてくれる人がいる」と常に確認したくなるのです。
これを解決するには、
- 日頃から、呼ばれる前に様子を見に行く
- 手が空いた時間は話し相手になるなど
寂しい状態を減らす対応を継続してみましょう。
来てくれると分かれば、むやみに呼ばなくなります。
認知症対応のポイント：
技術の進歩により膀胱に溜まったおしっこの量を把握できるようになりました。また各部屋に見守りカメラやマイクを設置することで介護者の負担が減ります。
こういった新しい介護の方法ができることで、人員不足が解決されていくのではないでしょうか？
もちろん導入には既存の圧力がありスムーズに取り入れられることはないでしょうが、技術の進歩という大きなうねりに対応できなければ、潰れてしまうだけです。
この問題は現場で扱える範囲を超えていますので、経営陣の賢明なる判断にお任せするしかありませんね。
