認知症者への基本対応
- 受容（否定しない、認知症者の世界観を受け入れる）
- 傾聴（自分のききたいことではなく、相手が話したいことを聞き出す）
- 怒らない（自己覚知によるアンガーマネジメントで対策）
トイレの便器で手を洗う事例：
デイサービスのケアマネから相談がありました。
デイ中に便器の水で手を洗うことがあり、家ではどうですか？との質問でした。
デイケア中は職員が付き添っているので便器の水で手を洗いそうになると声をかけるのですが、３回に１回は声かけを無視し便器で手を洗ってしまいます。
家ではひとりでトイレに入り、扉を閉めるのでそのような状況になっていることを知りませんでした。
認知症がそこまで進んでいることに驚きを隠せません。
トイレの便器で手を洗う原因：
・習慣
排泄後は手を洗う
・環境面
トイレの中に手洗い場がないもしくは、目につきにくい場所にある
・精神的不安
自分の家ではないので、環境に慣れていない、強迫観念的なもの
トイレの便器で手を洗わない対策：
トイレ内で普通に手を洗える環境にすることが大事だと思います。
トイレ内にゆとりがあれば手洗い場の設置工事をするなど、広さがなければ、手洗い付きのタンク便器に変更してもよいでしょう。
２４時間行動を見守ったり、抑制することはできませんので、便器で手を洗ったものとして考えたほうが良いでしょう。
もちろん、一緒にいるときは寄り添って見守り、適時声かけをしてください。
「ここで手を洗うのはダメ」
「ここは汚い」
と言ってみても、意味は伝わらず、あなたの怒っている感情だけが相手に伝わるだけです。
「せっけんで手を洗いに行きましょう」
「かがむと腰が痛いので、あっちで手を洗いましょう」
など、移動してもらえるように優しい言葉かけを工夫してみてください。
認知症対策ポイント：
・便器を手洗い一体型に変更
・家内の清潔・衛生の維持
トイレ内にタオルを設置
こまめに手洗いやウェットティッシュで手を拭いてもらう
手で触りそうな部分のアルコール消毒
